Here you will find information to get you started at Sunstone Psychiatric, including how to become a client, scheduling a first visit, what to bring to your first appointment, and insurance information.  For more details please review our Policies.

Initial Interest Form

To begin treatment with Sunstone Psychiatric the Initial Interest Form must be completed.  Once received, it will be reviewed by the clinical team to assure a good fit.  At this time, it will take us up to 2 weeks to reach out by phone to schedule an initial appointment or provide with appropriate referrals.  

Before Your Visit

Please make a list of all your current medications (including over the counter medications and herbal medications) and bring it with you to your first appointment. Some patients find it easiest just to bring their medication bottles.

Please bring your driver's license/state issued ID and insurance card to your first appointment as well. We will need to make a copy of both of these for our records.

Call your insurance carrier to confirm benefits and be sure you understand the costs associated with treatment.  

Take a moment to familiarize yourself with our practice policies, including cancellation, missed appointments, patient rights and responsibilities, as well as privacy practices.

Consider completing a release of information for current or past providers for continuity of care. Your clinician may also ask for you to complete a release for treatment planning or coordination of care purposes.
